Life is costly. For many of our patients, just getting enough to eat is often a financial hardship. At Sequoia Community Health Centers, living life well does not translate into hardship. Sequoia offers a sliding fee program to patients not covered by any form of public or private insurance on all services required for an office visit, including exam, lab work, x-ray and other procedures. This rate is among the lowest sliding scale co-pay in the entire San Joaquin Valley.Of course, Sequoia accepts Medicare, Medi-Cal, and private insurance as well. Plus, as a further commitment to our patients, Sequoia’s on-site eligibility workers enroll patients in Healthy Families, Medi-Cal, Access for Infants and Mothers (AIM), Children’s Health Initiative (CHI), Child Health Disability Program (CHDP), Family Pact and Cancer Detection Program (CDP) as well as assist patients to keep their enrollment status current. For Patients new to Sequoia:
New Patients should call (559) 457-5826 to make an appointment at any of our eight locations. The representative that you speak with will work with you to determine which of our Health Centers is most convenient for you and schedule your initial appointment.
Two days prior to your appointment we will try to reach you to complete your Pre-Registration process. At this time, we will gather your insurance information, confirm address, and remind you of what you need to bring to your appointment. We may also ask questions to determine your eligibility for any of our specialty programs.
If we do not reach you 2 day before your appointment, we will try to call again the day before the appointment to remind you of the date and time of the appointment and to confirm that you will be able to keep the appointment or to reschedule if needed.
New and existing patients (adult and pediatric) should bring the following for all appointments:
• Insurance Card or Proof of Income
• Photo ID
• Immunization record
• Proof of address such as PG&E or other bill
• Co-payment or Share of cost
All new patients to Sequoia should arrive 30 minutes before their appointment time to complete the registration process. Existing Sequoia patients should arrive 15 minutes before their appointment time to complete their registration update.
